Job summary

Larwood Health Partnership is looking for a forward thinking, enthusiastic Clinical Practitioner to join our already established team to continue to develop our ways of working to provide timely, quality care for our patients. We are based over 4 sites with a patient list size of almost 35,000 and rated Outstanding by CQC. This has resulted in new ways of working for the Clinical/Nurse Practitioner Team to improve patient care with our GP Practices.

Main duties of the job

The applicant needs to have experience in the management of minor illness/injury with a qualification at Level 7 such as the APACs course (Advanced, Physical and Consultation Skills module) or minor illness course. Experience of chronic disease management is also desirable for this post. The Practitioner will be part of the Acute Care Team that will assess patients for urgent care on the day which may take the form of seeing patients face to face or by video consultation. The applicant must already have/ currently undertaking the non-medical prescribing training.

Job description

Job responsibilities

    • Assess, plan, develop, implement, and evaluate treatment programmes that promote health and well-being.
    • Consult with patients presenting for acute minor illness assessment, treatment, and diagnosis at point of first contact within the ACT setting.
    • Assess, plan, implement, evaluate, and review individual treatment plans for patients with a known long-term condition.
    • Each individual will have a half day within their working week for their own speciality, to be decided with the individual at time of employment which will be beneficial to the needs of the patient population. This will be documented within the induction folder or professional development plan to be kept in your personnel folder.
    • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
    • Undertakes annual review of patients with chronic disease within the primary care setting and community as identified.
    • Undertake medication reviews in line with practice protocol as identified, face to face or remote as appropriate with patient.
    • Initiate tests and follow through results by assess to path links.
    • The Practitioner must be appropriately trained in radiation awareness and adhere to the Imaging referral protocol for nurse practitioners when referring patients for radiological tests.
    • Make professional autonomous decisions for which he/she is accountable.
    • Work with other health care professionals to diagnose, monitor, manage and treat conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
    • Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
    • Work with patients in order to support adherence to prescribed treatments.
    • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
    • Work within the non-medical formulary in collaboration with GPs and other clinicians to meet the needs of patients.
    • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
    • Support patients to adopt health promotion strategies that promote patients to live healthily and apply principles of self-care.
    • Support and manage health needs of women presenting for: emergency contraception /sexual health needs.
    • Implement and participate in vaccination and immunisation programmes for both adults and children.
    • Promote and deliver evidence-based care for patients presenting with aural conditions.
    • Meet the needs of patients presenting for wound management.
