Job summary

We are looking for a Practice Nurse to join our friendly, forward thinking Practice based in Liscard, Wirral.

We are looking for a candidate who can carry out general Practice Nurse tasks, such as vaccinations, smears and chronic disease management.

We require someone who has an interest in diabetic management for our patients. We are happy to provide training in diabetes, if needed.

We are flexible on days/hours to be worked and we are looking for someone to work 16-24 hours per week.

Main duties of the job

The main tasks for this role include vaccinations, chronic disease management, QOF work, travel vaccinations and general nursing duties. Training can be provided if necessary.

An interest in diabetes is essential but again we can provide training if needed.

We are a friendly Practice based in Liscard, Wirral.

We have a Practice size of approximately 5800 patients.

We have 4 GP Partners, Salaried GP's, Practice nurses, reception/administrative staff and a Practice Manager. We are also a training Practice and currently have ST1 and F2 doctors.

We are supported by Wallasey Wellbeing PCN who provide Paramedic, pharmacist and home visiting/care home support.

Job Summary:

Responsible for the delivery of all practice nursing services care to the practice population. Delivering care within the boundaries of the role with a focus upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. Working collaboratively with the general practice team to meet the needs of patients, and supporting the delivery of policy and procedures

Main responsibilities:

  • Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being
  • Implement and evaluate individual treatment plans for patients with a known long-term condition
  • Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
  • Support and advise women requesting information relating to family planning needs
  • Implement and participate in vaccination and immunisation programmes for both adults and children
