Job summary

Mendip Vale Medical Group are looking for an experienced manager to support our Social Prescibing team in North Somerset & Bristol to deliver safe and effective care to our patients.

The post-holder will provide leadership and management skills to the Social Prescribers in Mendip Vale Medical Group (MVMG),meetings agreed aims and objectives within a profitable, efficient, safe and effective working environment. Working with the Divisional Directors, the post-holder will be responsible for promoting healthy living to our patients, by providing expert signposting and support to engage patients with a wide range of community services.

As the Social Prescriber Manageryou will motivate and inspire the team, ensuring the highest standards of customer service are delivered.

Main duties of the job

The Social Prescriber Manager will develop good working relationships with:

  • Patients

  • Community services, 3rdsector

  • Cliniciansandthe widersurgery staff

  • Key stakeholders across the whole of BNSSG

Based on guidance from the referrer you support your team to identify a range of options that could assist patients by improving their independence, health and well being by producing a simple personalised care and support plan.

You will be responsible for identifing, monitoring, reviewing and addressing issues across MVMG that impact on the Social Prescribing Team. and will be required to lead the Social Prescribersby facilitating a collaborative, transparent, and knowledge-sharing environment.

The role will also require you to assist in the development of relevant policies and procedures to ensure services are delivered in a safe, caring and effective way.

You will be working with a friendly and knowledgeable team, working in conjunction with the Partners, and Divisional Director to provide day to day supervision and management of the Social Prescriber team and operational service delivery. As a high-achieving and well-organised practice, we embrace a Multi-Disciplinary Team approach to care provision.

The Mendip Vale team work together across 8 sites in North Somerset & Bristol, looking after around 80,000 patients across a semi-rural location in North Somerset and urban setting in Bristol and South Gloucester. Changes made from merging and working more effectively have enabled us to make investment into our buildings, making our estate one of the newest and brightest in the South West.

Other Responsibilities:

  • To support the Area Manager with delivery of the KPIs and ensure these are understood by the Social Prescribers.

  • To oversee staff resources and daily work allocation of the teams within the budget set with the Head of Operations

  • To ensure all staff appraisals, mandatory training and personal development plans are completed within the timescales set.

  • Working with the Communications and Engagement Manager so regular communications updates which are understood by the staff are cascaded

  • To undertake any other duties commensurate with the post holders role as agreed with the Head of Operations

  • To oversee the delivery of services ensuring that there is fairness of workload and adequate availability.

Communication

  • Able utilise a full range of communication methods to disseminate information to the team

  • Able to y adapt communication style and content to reflect different type of people when making contact.

  • Ability to receive sensitive or contentious information and process appropriately; and where needed translate this information into a delivery message or plan for the wider team.

  • Lead in positivity and effective working relationships with commissioners, other health care providers and other key health care providers and other key partners to support an integrated approach to high quality patient care.
