Job responsibilities
The post-holder will look to improve patients
health outcomes and the efficiency of the Mendip Vale Medical Group (MVMG) by
providing direct, accessible and timely medicines expertise. To pro-actively
transfer workload relating to medicines optimisation issues from other clinical
staff to improve patient care, safety and free up scarce clinical staff to
spend more time on clinical care. You will support the Practice in delivering a
safe, high performing; effective, sustainable, innovative person centred medical
service, and lead the dispensary team.
The post-holder will develop good working
relationships with:
Patients
GP,
nurses and other practice staff
Other
local practices clinical pharmacists
Community
pharmacists and support staff
Hospital
staff with responsibilities for prescribing and medicines optimisation
Members
of the CCGs medicines management team.
Clinical Responsibilities:
To undertake a
variety of duties, including working
across the practices in the locality as lead for the medicines management
processes.
Working across
sites to optimise processes to achieve highly effective, safe and patient
centred prescribing.
To work closely
with the Senior Clinical Pharmacist in the delivery of our business priorities
with a practice-based focus.
Working towards
or as an independent prescriber, to undertake patient consultations with a
focus on groups of patients with potential medication issues, whilst prescribing in accordance with the practice prescribing
formulary (or generically) whenever this is clinically appropriate.
Recording clear
and contemporaneous consultation notes to agreed standards.
Recording
appropriate Read Codes to maximise the practice QOF performance, and
participate in QOF recall reviews.
Collecting data
for audit purposes.
Compiling and
issuing computer-generated acute and repeat prescriptions (avoiding
hand-written prescriptions whenever possible).
Other Responsibilities:
Awareness
of and compliance with all relevant practice policies/guidelines, eg
prescribing, confidentiality, data protection, health and safety
To undertake any
other duties commensurate with the post holders grade as agreed with the
Senior Clinical Pharmacist and/or practice lead GP.
To maintain
registration as a pharmacist and comply with appropriate professional codes
To develop and
facilitate a good working relationship with community pharmacists and other
local providers of healthcare
Support the Senior
Clinical Pharmacist in the commercial management of the dispensary to support
cost effective medications and obtain best value within the budgets available
A
commitment to life-long learning and audit to ensure evidence-based best
practice
Contributing
to evaluation/audit and clinical standard setting within the organisation
Contributing
to the development of computer-based patient records
Contributing
to the summarising of patient records and Read-Coding patient data,
contributing to the maximum practice performance under QOF.
Attending
training and events organised by the practice or other agencies, where
appropriate.
Communication

Confidentiality
- In the course of
seeking treatment, patients entrust us with, or allow us to gather,
sensitive information in relation to their health and other matters. They
do so in confidence and have the right to expect that staff will respect
their privacy and act appropriately
- Whilst performing the
duties outlined in this Job Description, the post-holder may have access
to confidential information relating to patients and their carers, surgery
staff and other healthcare workers. They may also have access to
information relating to the Surgery as a business organisation. All such
information from any source is to be regarded as strictly confidential.
- Information
relating to patients, careers, colleagues, other healthcare workers or the
business of the Surgery may only be divulged to authorised persons in
accordance with the Surgery policies and procedures relating to
confidentiality and the protection of personal and sensitive data.
