Job summary

Ribbleton Medical Centre are expanding their practice team and looking for a Full Time Clinical Pharmacist. We are very flexible with working hours.

The post holder will work within their clinical competencies as part of a multidisciplinary team to:-

  • Provide expertise in clinical medicines management
  • Provide face to face structured medication reviews,
  • Manage long term conditions
  • Management of medicines on transfer of care and systems for safer prescribing
  • Manage repeat prescription authorisations and reauthorisations
  • Acute prescription requests

Main duties of the job

Patient facing medicines support

  • Hold clinics for patient requiring face to face structured medication reviews. Provide a telephone support for patients with questions, queries and concerns about their medication.
Medication review
  • Undertake structured medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes and order relevant monitoring tests.
Risk Stratification
  • Design, development and implementation of computer searches to identify cohort of patients at risk of him from medicines.
Long term conditions clinic
  • See patients in multi-morbidity clinics.

Repeat Prescribing

  • Produce and implement a practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat medication.
Medicines safety and quality improvement
  • Conduct audits and improvement projects

Ribbleton Medical Centre is a three partner practice with a list size of 8800 patients and growing.

We are a training practice and host Medical Students, Foundation Year Doctors and Registrars.

Our team consists of 3 GP Partners; 3 Salaried GPs; 2 Clinical Pharmacists; 1 Nurse Practitioner; 1 Physician Associate; 3 Practice Nurses; 2 Medicines Management; 1 Practice Manager; 1 Assistant Manager; 9 Receptionists/Admin Assistants.

Key Duties and Responsibilities:-

  • Patient facing medicines support - face to face structured medication review.
  • Medication review - medication review with patients with multi-morbidity and polypharmacy.
  • Risk stratification - design, develop and implement computer search to identify cohort of patients at high risk of harm from medicines.
  • Long term conditions - review patients with long term conditions.  Review the need for each medicine, a review of monitoring needs and support with medicines taking.
  • Unplanned hospital admissions - Identify patients most likely to be at risk of hospital admission.  Manage medicines related risk for readmission and patient harm.
  • Repeat prescribing- manage the repeat prescribing reauthorisation process.
  • Triage - ensure patients are referred to the most appropriate clinician for the appropriate level of care within an appropriate time frame.
  • Medicines safety and quality improvement- identify and provide leadership on areas of prescribing requiring improvement.
  • Care Home medication reviews - manage own caseload of care home residents.  Undertake clinical medication reviews with patients with multi-morbidity and polypharmacy. Work with care home staff to improve safety of medicines ordering and administration.
  • Domiciliary clinical medication review - manage own caseload of vulnerable housebound patients at risk of hospital admission and harm from poor use of medicines.
  • Service development - Develop and manage new services that are built around new medicines or NICE guidance.
  • Cost saving programmes - make recommendations for and manage Pharmacy Technicians to make changes to medicines to save on medicine costs.
  • Management of medicines at discharge from hospital - to reconcile medicines following discharge from hospital, intermediate care and into care homes.  
  • Implementation of local and national guidelines and formulary recommendations.
  • Medicines safety - manage MHRA alerts, product withdrawals.
