Job summary

Whilst this is a primary care-based role, primary care-based experience is desirable but not essential. What is key to this role is the co-ordination, project management, rapid implementation of new roles and systems and the strategic and operation leadership to lead change and teams.

You will be the first point of contact for the network and act as an enabler for improved health to improve services to City Centre Network.

To provide management and organisational support for the networks to facilitate growth of the PCN and the delivery of services in line with the NHS England strategic development plans, GP contract and Network Contract Directed Enhanced Service Specification(s) DES.

The post holder will contribute to the successful establishment, running and ongoing development of the network,

The post holder, guided by the Clinical Directors, will ensure that the network and its member practices have clear lines of accountability and governance.

Main duties of the job

· Managing the finances of the network, sorting payments, liaising with PCS, preparing budget reports on behalf of the PCN

· Monitor the performance of the PCN and report into CD/Board any deviation from network plans

· Develop an annual network plan and future goals for the network

· Liaise with the CDs and discuss the network work plans

· Chair & prepare key meetings

· Liaise closely with the network practice managers and proactively engage with and understand their individual practice needs

· Link with the third sector agencies and other key stakeholders, facilitating joint working and projects

· Lead and manage key work programmes for the network, e.g., vaccination programme, extended access

· Link in with other employers and co-ordinate the ARR deployment on behalf of the network as well as workforce declaration returns

· Manage the network shared directly employed ARR workforce, and work with the network to come up with HR/employment solutions

· Keep abreast of the network DES requirements and ensure that the network is fulfilling the responsibility of the contract

· Identify potential sources of funding to support the networks and its plans and submit bids as appropriate.

About us

City Centre/SHU Primary Care Network is a group of 5 GP practices with a population of 56,000 patients. This is a diverse population including a large number of students and a significant number of people in care homes. We also work with the majority of people who are homeless and rough sleeping in.

The post holder will work flexibly across our network practices and closely with the Clinical Directors and will support practices to achieve improved outcomes for the local population.

The post holder will manage the network funding and the collection of data and information to ensure robust budget monitoring, payment, and provision of assurance to the network and/or commissioners relating to any contract compliance.

Primary responsibilities

The remit of this role is broad and evolving and will require attention to detail in the delivery of high-quality day to day operations within the network.

· Act as a champion for the network and encourage and support all practices in the network to engage in the process.

· Work with practices within the PCNs to identify areas of common concern or potential collaborative working to improve the resilience of general practice and sustain local services as well as identify and meet the support needs of the practices in the network.

· Facilitate the development of an honest and transparent culture across the network which support the members to grow and flourish in the improvement of services for both staff and our patients.

· Work with and support the Clinical Directors to develop and deliver their PCN Plans and performance requirements and implement any remedial plans where necessary.

· Continue to support and encourage learning to continually improve network services and patient outcomes.

· Attend relevant meetings to ascertain a wider knowledge of proposals for change in terms of delivery of health care systems across the practices and feedback as appropriate.

· Ensure all relevant national strategies, policies, targets, and frameworks (that are agreed by the network groups) are fully implemented.

· Represent the PCNs at meetings alongside the Clinical Directors.

· Support practices with formulating project and/or delivery plans in relation to meet the GP contract/DES specification, including writing plans and business cases on behalf of the network as appropriate.

· Support the review of PCN agreements as and when required including reflecting changes to practice members and the GP contract/DES specification(s) as these are issued by NHS England.

· Undertake a lead role, as required, to ensure the Network complies with relevant and legislative responsibilities relating to GDPR, Health and Safety and Human Resources.

· Ensure that patient safety is at the forefront of all service developments and delivery.

· Organise meetings including Board and steering group and others as they arise, minute taking and progressing any action points.

· Organise PCN events.

· Identify potential sources of funding to support the networks and its plans and submit bids as appropriate.

· Ensure that specifications, service contracts and/or any plans are adequately monitored and on schedule and that this information is appropriately communicated to relevant stakeholders.
