Job summary
We are looking to recruit an Advanced Nurse Practitioner to join our acute clinical team.
The successful candidate will be an experienced senior nurse educated to Masters Level, an independent
and experienced prescriber who is able to work autonomously whilst thriving in
a busy team. You will have the opportunity to work with our extensive multi-disciplinary team at the practice and access a wide range of additional roles through our PCN, to include Pharmacists, First Contact Physio, Acute Visiting Practitioners, Social Prescribers, Mental Health Practitioners and Dietician.

Main duties of the job
The post holder must be an experienced
senior nurse who, acting within their professional boundaries, will provide
care for the presenting patient.
They
will be able to demonstrate autonomous practice in the assessment, diagnosis, treatment,
including prescribing, for patients with multi-dimensional problems and be educated at Masters Level in Advanced Practice and have been assessed as competent at this level.

MAIN DUTIES AND RESPONSIBILITIES
Clinical responsibilities
- Work autonomously and be accountable for their own professional actions.
- Undertake telephone advice calls.
- Undertake clinical nursing practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.
- Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions.
- Within their scope of practice, take an appropriate history, conduct physical examinations, formulate differential diagnoses and carry through management treatment plans following local and National guidance ensuring relevant follow up.
- Has the authority to request, where indicated, appropriate diagnostic tests / investigations using judgement and clinical reasoning, based on differential diagnoses.
- Prescribe medication and treatment in accordance with NMC guidelines for Independent non-medical prescribers and the ICB formulary.
- Report, discuss and refer areas on clinical uncertainty with the Duty Hub GP, secondary care or partner organisations.
- Review and action patient letters/discharges/results.
- Respond to administrative/clinical queries via EMIS.
- Undertake Home Visiting as needed.
Communication
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
- Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
- Utilise and signpost to supporting services when appropriate.
- Maintain effective communication within the practice environment.
- Attend and contribute to practice and primary care team meetings.
- Deliver a quality service within the Practice
- Recognise and work within own competence and professional code of conduct as regulated by the NMC.
- Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
- Work as a valuable team member within the greater practice team.
- Deliver care according to practice policies, local guidelines, NICE guidelines and evidence-based care.
- Consider and aim to deliver cost-effective prescribing, working towards practice, ICB and national prescribing targets.

Professional Development
- Maintain clinical currency and CPD according to NMC, prescribing and CQC guidelines.

- Participate in an annual individual performance review, taking responsibility for maintaining a record of their own personal and professional development and NMC revalidation.
