Job summary

Greenwell PCN comprises of seven Practices located in Greenford and Hanwell. The practices have a combined geographic area made up of around 39,000 patients. We are looking for Advanced Clinical Practitioner to join our enthusiastic team of clinicians and administrators. The role will contribute to the improving quality of care of our patients across the network of practices in our community

Main duties of the job

The Advanced Clinical Practitioner will act as an autonomous advanced practitioner who is able to work independently and in conjunction with other members of the clinical team. The post holder will be an experienced caring Nurse who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries.

Greenwell Primary Care Network is an NHS Collaboration between 7 GP Practices in Greenford and Hanwell Oldfield Family Practice, Mansell Road Practice, Eastmead Avenue Surgery, Elmbank Surgery, Greenford Avenue Family Health Practice, Hanwell Health Centre and Westseven GP. We have an excellent relationships across the 7 practices and are working collaboratively to provide extended services to address any inequalities and to provide great care to our patients.

Job description

Job responsibilities

The Advanced Clinical Practitioner will be based at one of the practices within the Network whilst delivering a service to patients registered at all the practices.

The Advanced Clinical Practitioner will be able to demonstrate safe, competent clinical decision-making and deliver expert care, including assessment, diagnostic skills, treatment and evaluation of patients. Working in the community; visiting patients at home, you will deliver holistic care, managing complex cases.

Key priorities

Make professionally autonomous decisions for which he or she is accountable, to include making differential diagnosis using critical thinking, clinical decision making and problem-solving skills.

Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

Order necessary investigations; interpret results and provide treatment and care both individually, as part of a team and through direct referral to other primary and secondary services.

Manage and meet the needs of patients diagnosed with primarily conditions including dementia, COPD, Diabetes and heart failure.

Work closely with the Social Prescriber to deliver a seamless experience for the patient.

Provide with the patient an evidence-based management plan that includes evaluation and review where appropriate and incorporates cultural preferences, health beliefs, behaviours and traditional practices.

The post-holder will focus on the most complex patients within the network, treating patients within their homes.

The post-holder will work with a priority group of elderly patients, those with chronic disease and conditions.

The post holder will work within the integrated network team, alongside community services and social services to create appropriate care plans.

Liaise where appropriate with other members of the PCN team in order to provide expert care for patients and promote shared decision making.

General Duties

To deliver high quality, compassionate care and treatment to patients with a focus on promoting health, self-care and independence using a personalised and holistic care approach.

To contribute to local and national practice targets e.g. QOF, prescribing incentive scheme, national benchmarking etc.

To see extra or emergency patients where necessary.

To ensure all specimens (cytology, blood, urine etc.) are taken in accordance to protocols, recorded into patients records and labelled ready for collection.

To take responsibility for own development, learning and performance to ensure own educational commitment is at least sufficient to maintain registration requirements.

To act with professionalism and ensuring that ethical conduct is adhered to at all times.

To participate and promote child and vulnerable adult safeguarding and to extract safeguarding information and follow the appropriate pathway when required.

To keep the working area clean, tidy and professional whilst adhering to GDPR regulations.

To work across different practices on an ad-hoc/when required basis.

It will be necessary to attend and contribute to various practice meetings as requested.

  • Masters or working towards a Masters for Advanced Clinical Practitioner Qualification
  • Qualified Independent Nurse Prescriber on the NMC register

  • Meets the standards for registered ANP working at advanced level
  • Ability to record accurate clinical notes
