Job summary

  • We are looking for a Clinical Pharmacist to join our innovative multi-disciplinary team for a period of between 9-12 months maternity cover from August 2023. This is an exciting opportunity to gain much sought after primary care experience and be part of a supportive, well-established, and forward-thinking General Practice. You will work alongside our growing primary care team including a pharmacy technician and another full time clinical pharmacist to provide medicines management support to our practice population. Ideally the post holder will be a non-medical prescriber but all applications will be considered.

Main duties of the job

To update prescriptions and changes to records in response to written requests on prescribers instructions. To consult with patients F2F or by phone providing support on medicines management queries/requests and liaise with community pharmacies to improve practice prescribing systems and costs.Undertake medicine reviews and make recommendations for changes to a prescriber or independently if a NMP.To reconcile medicines following discharge from hospitals, intermediate care. After the GP has approved the medication changes; perform a clinical medication review; produce a post-discharge medicines care plan including dose titration and booking of follow-up tests..Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in care homes).Actioning clinical outputs by phoning / contacting patients to inform about results and /or changes requested by the doctors.Answer medicine-elated enquiries from GPs, other practice staff and patients with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes. Focus on medicine availability and changing to alternative medicines when there are supply problems. Provide a telephone help line for patients with questions, queries and concerns about their medicines. Undertake risk assessment and management and ensure compliance with medicines legislation.

About us

Grosvenor is an innovative practice across 2 sites caring for 15,000 patients forming part of the Grosvenor, Hungerford, Ropegreen Primary Care Network providing a strong foundation for high-quality place-based care in the South of Cheshire. The Practice has 5 GP partners, 2 Salaried GPs, 2 ACPs, 1 Paramedic trainee ACP, 2 Clinical Pharmacists, 1 Pharmacy Technician, a well-established Practice Nurse team including HCAs and GPAs and a well-being team of 2 care coordinators, social prescriber and a mental health nurse. We work closely with the community teams and have first contact MSK practitioners working within the practice. The Practice fosters a strong ethos of mutual support and teamwork meeting regularly to discuss clinical cases alongside structured educational meetings. We are currently supporting an ACP and TNA Apprentice and our clinical model supports mentorship and professional development. The Practice hosts one of 2 locality training hubs in Cheshire supporting the development of Primary Care as a learning environment for all multi-disciplines. We are continuously contributing to the development of new ways of working across Primary Care, including hosting the weekend Hub for South Cheshire patients and working smarter with other practices within our local Care Community in supporting community projects such as the homeless health and well-being clinics. We welcome informal discussions and visits to our Practices.

Your responsibilities are but are not limited to;

To work with GP Partners and staff to enter prescriptions and changes to records in response to written requests on prescribers instructions

Familiarise yourself with practice routines and working methods for prescribing and some practice systems (e.g. Near Patient Testing). Make suggestions and implement changes by agreement.

Understand QOF and practice systems. Work with Partners and staff to consult with patients in surgery or indirectly to update clinical parameters and QOF registers

Work to improve QOF performance

Be available to staff and doctors to provide support on medicines management and Liaise with community pharmacies to improve practice prescribing systems and costs.

  • Familiarise yourself with the pharmacy needs of the practice and patients
  • Undertake patient consultations for minor illness and prescribe as appropriately for same day appointments under close supervision
  • Undertake medicine reviews and update medicine reviews under close supervision
  • Undertake surgeries of same day appointments as independent practitioner
  • Undertake medication and disease reviews including QOF reviews as independent practitioner

Work with the practice manager and GPs to ensure the practice is compliant with CQC standards where medicines are involved. Undertake risk assessment and management and ensure compliance with medicines legislation.

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).
