Job summary

Bridge Cottage Surgery are looking for a full time or part time Practice Nurse or Nurse Practitioner to join our supportive and friendly team of nurses with opportunities for self development.

Bridge Cottage Surgery is a semi-rural practice of approximately 16,000 patients based in Welwyn, Hertfordshire. The surgery has 7 GP Partners, 3 Associate Clinicians, Pharmacist and a nursing team of 4 nurses and an HCA. Bridge Cottage Surgery is easily accessible off the A1M and has excellent transport links in and around Hertfordshire and is only 20 minutes from London.

To provide nursing services to a defined practice population with the assistance of, and in collaboration with the Lead Nurse and GP colleagues.

Main duties of the job

This role will include clinical duties: ECGs, dressings, BPs, Ear irrigation, injections, cervical screening, restocking and ordering. To contribute in the development of services provided by the Practice and to participate in clinical audits. To work with the GPs to develop and implement the Practice Development Plan, the National Service Framework targets, Key Performance Indicators and to meet and maintain the quality indicators contained within the Quality and Outcomes Framework.

Job description

Job responsibilities

1. To work with the Nursing team to assess, plan and provide health care and nursing services to the practice population, ensuring they are effective, relevant, evidence-based and of a high standard

2. To work with the Nursing team to provide a comprehensive general practice nursing service, to include wound care, urine tests, childhood immunisations, seasonal influenza, pneumonia, shingles, rotavirus immunisations, NHS assessments, ECGs, blood pressure, travel immunisations and advice, within the surgery and where appropriate in patient homes, reviewing blood test results and taking action where required.

3. To provide a womens health service, to include cervical screening and advice and information regard breast screening, Chlamydia testing and family planning

4. To with the Nursing Team & GP colleagues to achieve and maintain cervical screening targets. To record and monitor result, including efficient and accurate call and recall systems.

5. To work with the Nursing team to provide an effective health promotion service, to include smoking cessation and healthy lifestyle clinics.

6. To work with child health, to include the administration of the childrens immunisation programme according to current schedules, the accurate recording of the information in the child health records and to participate in measures (such as education sessions) to improve the uptake of childhood immunisations, especially the Measles, Mumps and Rubella vaccine, whooping cough in pregnancy.

7. To work with the practice team in order to organise, implement and administer the annual influenza and pneumococcal vaccination programme, to include the identification, targeting and immunisation of vulnerable patients within the practice population, to ensure that local and national targets are met.

8. To work with the Nursing team to identify and maintain registers of those patients with chronic disease such as asthma, chronic obstructive pulmonary disease (spirometry), diabetes, thyroid disease, epilepsy and coronary heart disease, hypertension.

9. To assist in the development, implementation, delivery and evaluation of programmes of care for those patients with chronic disease as dictated by the National Service Frameworks (NSFs) targets and to meet and maintain the quality indicators laid down in the nGMS contract initiated in April 2004

10. To ensure that professional responsibilities are met as practitioner supplying and administering medicines in accordance with a Patient Group Direction.

11. To assess patient understanding of treatment proposals, gain valid informed consent and works with the NMC Code of Professional Conduct to provide care for patients who lack the capacity to consent to treatment.

12. To use a range of verbal and non-verbal communication tools, to communicate effectively with patients to deliver effective healthcare. This will include patients who have special communication needs, e.g., those patients who are sensory impaired or who have mental health issues.

13. To work with the practice team to ensure clinical governance standards are met, including managing clinical risk within own clinics.

14. To work closely with the practice team, primary care and community nurses, allied health professionals and social services colleagues by active participation in clinical meetings, to ensure effective lines of communication in order to achieve high standard and continuity of care for the practice population.

15. To work with the Nursing team and GP colleagues to develop local enhanced services, if required.
