An exciting opportunity has arisen for an experienced self-motivated pharmacy technician with excellent communication and interpersonal skills to join our forward thinking GP Practice in Surrey. Cranleigh Medical Practice sits in the picturesque village of Cranleigh approximately 8 miles South-East of Guildford, and has a patient list size of approximately 16, 250.

Main duties of the job

As our first pharmacy technician you have a unique opportunity to contribute to the development of the role. You will join a really supportive practice and work as an active member of the Multi-Disciplinary Team, which includes GPs, Nurses, Paramedic and Support Staff to ensure the delivery of a legal, safe, high quality prescribing which reflects National and local policy and priorities. You will have the opportunity to develop relationships and links with other pharmacy professionals, across the Primacy Care Network (PCN) and Clinical Commissioning Group (CCG) and build a network for professional peer support.

Job responsibilities

The duties and responsibilities to be undertaken may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Practice Manager or Prescribing Lead, dependent on current and evolving Practice workload and staffing levels:

  • Provide hands on support and advice to the clinical team to promote and influence high quality prescribing to minimise risk and promote safe and effective patient care
  • Work closely with the admin team to ensure coordinated and efficient processing of prescription queries
  • To respond to medication queries from patients and staff in a professional manner referring to the appropriate clinician, when required, in accordance with practice protocols and to promote shared decision making.
  • Provide specialist expertise, where competent, to address both the public health and social care needs of the patient, including lifestyle advice, and service information.
  • Monitor adherence to the Practice repeat prescribing policy and oversee the processing of repeat prescriptions; including supporting the increase in use of Electronic Prescription Services (EPS) and reviewing patient suitability for Repeat Dispensing
  • Work with clinicians in accordance with the Practice medicines management operating policies and procedures
  • Work alongside clinicians to carry out medicine reconciliations for discharge summaries, clinic letters and other transfers of care information and communication, in line with Practice policy
  • Develop positive and professional working relationships with colleagues in local pharmacies to ensure good patient care
  • Work collaboratively to ensure that all medicines management actions and interventions are communicated to and shared with all relevant parties, staff and stakeholders
  • Monitor and action safety and drug alerts in a timely and appropriate manner, reporting to the Prescribing Lead and/or CCG as appropriate.
  • Update and maintain accurate and comprehensive patient medication records including medicines supplied by prescribers other than the GP e.g. Hospital only medicines
  • Liaise with primary and secondary care colleagues and others to resolve prescribing queries, referring to the prescribing lead or practice manager where a resolution has not been possible
  • Coordinate appropriate monitoring of high risk medication in accordance with Practice policy
  • Work alongside clinicians and Operations Manager to develop and maintain a robust recall system to ensure medication reviews are completed in a timely manner
  • Work with clinicians to develop and undertake medicine review audits and other quality improvement/ project work as required
  • Support initiatives for Antimicrobial Stewardship to reduce inappropriate antibiotic prescribing
  • Undertake regular analysis of prescribing data and provide Practice updates
  • Contribute to Practice meetings and educational sessions as identified
  • Work to support the achievement of prescribing outcomes for Quality Outcomes Framework and Manchester Standards
  • To assist in the control of medicines stocks within the practice, ensuring compliance with medicines legislation
  • Develop relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team to support integration of pharmacy across the Guildford & Waverley Integrated Care Partnership (ICP); including monitoring adherence to the local formulary
  • Undertake any other additional duties appropriate to the post as requested by the Partners or the Operations manager
