Are you looking to join an award winning, innovative and inspiring Practice. Concord Medical Centre is growing fast following local new housing developments and we are now looking after over 17500 patients. We are now looking to recruit an excellent GP to join our great team.
Concord Medical Centre is a modern purpose built Practice with excellent patient and staff facilities. We are ideally located near to the M4/M5 interchange in South Gloucestershire and offer free on site car parking for all our staff.
We are a teaching & training Practice with our own Research Hub and we actively encourage specialist interests and personal development.
Main duties of the job
We want you to enjoy a comfortable work-life balance so we can offer a range of flexible working patterns. We are working towards providing 15-minute consultation times and will support you to develop your career in the way that you want. Home visit rates at the practice are low.
On joining us you will have a comprehensive induction programme so you feel confident getting to know our wider team and how we run before seeing patients. An experienced GP mentor will be there for advice and guidance when you need it during your first year and beyond if required.
Relationships matter, so we make sure that there is protected time to attend regular meetings, including daily morning coffee with colleagues and more formal clinical professional development sessions.
Well established use of AccuRx, Floreys and video consultations and other healthcare innovations are actively sought.

At Concord Medical Centre we support and value our practice team in
delivering great patient care.
Our team is supported by 8 Partners including 6 GP/Partners, a Clinical Pharmacist Partner and a Nurse Partner. Our clinical team is diverse from Paramedics, Clinical Pharmacists, Pharmacy Technicians, Nurse Specialists, Social Prescribers and Wellbeing Coaches. The Clinical Team is well supported by our valued team of Patient Co-Ordinators and managers who minimise the administrative burden.
We are a teaching, training and research practice
working closely with the University of Bristol and UWE, our PCN, and One Care our vigorous
GP provider federation.
We offer informal visits to the Practice so that you can see what we do and meet some of the team. Please email the Practice Manager who will arrange.

Job responsibilities
Utilise the
Ask My GP interface offering Face to Face consultations, telephone
consultations, e consultations, visiting patients at home, checking and signing
via EPS repeat prescriptions and dealing with queries, paperwork and
correspondence as required.
Our systems promote meeting our patients needs and developing a more team centred approach to managing this. Our work on the Concord Project is helping us change our focus back to improving health outcomes.
Make
professional, autonomous decisions in relation to presenting problems, whether
self-referred or referred from other health care workers within the
organisation
Screen
patients for disease risk factors and early signs of illness
Cover all the
clinical management of the patients in surgery as appropriate including
initiating investigations, reviewing results, making referrals to secondary
care or to other providers as appropriate
Develop care
and treatment plans in consultation with patients and in line with current
Practice disease management protocols as well wider guidance such as that from
NICE
Ensure
appropriate follow up of patients
Record clear
and contemporaneous computerised consultation notes to agreed standards
Collect data
for audit purposes
Compile and
issue computer-generated acute and repeat prescriptions avoiding hand-written
prescriptions
Prescribe
generically whenever appropriately and in accordance with the Practice
prescribing formulary, NICE guidance and Good Prescribing Practice (as defined
by British National Formulary guidance)
Work with and
support other members of the primary health care team in delivering high
quality primary care
Play a lead
role in the practice achieving agreed QOF and other key performance indicator
targets
Develop
cohesive relationships with the Practice Team, Allied Health Professionals and
patients
