Job summary

Concord Medical Centre is growing fast and expanding the range of services available to our patients and our community.

We are looking for a newly qualified Nurse to join our award winning Practice to explore (with us) innovative ways of providing healthcare that improves patient outcomes and reduces inequalities. This is a new role and opportunity to work within Primary Care for a year in a fully supported role.

Main duties of the job

Due to funding from NHS England, we are delighted to offer the New to Practice programme as part of your employment. 

This programme has been specifically designed by the Avon Local Medical Committee (LMC), BNSSG Training Hub and Integrated Care Board (ICB) for nurses joining General Practice for the first time, this will be led by 2 legacy nurses who have a wealth of General Practice knowledge and experience and are well placed to support you as you transition into your new role.

The programme includes the following elements: we recognise that you may have transferrable knowledge and skills therefore we can tailor the offer to meet your individual needs:

  • 4 hours of protected CPD time per week (pro-rata)
  • BNSSG system-wide preceptorship programme for newly qualified nurses
  • Mentorship for a minimum of 1 hour per month with legacy nurse
  • Peer support – 1 hour per month peer support session providing the opportunity for you to network with other new to practice/newly qualified nurses.
  • Access to training
  • Clinical supervision provided in practice
  • Dedicated support from legacy nurses, Avon LMC director of nursing & ICB GPN lead

About us

We are a Training and Learning Practice with strong links to the University of Bristol, University of the West of England and the National Institute of Health Research.

Our Clinical Team is diverse with GPs, Nurses & HCAs, Pharmacists & Technicians, Paramedics, Physios and we are fortunate to have a Pharmacy within the premises offering additional services to help manage patient demand. We have a large supporting administrative and management team which include care co-ordinators and social prescribers.

We focus on communication - involving and engaging with our teams and our patients to continually learn and get better at what we do.

General Practice is busy but we work hard on the wellbeing of the team and the working environment.

Job description

Job responsibilities

Main Duties

Organise and co-ordinate the provision of nursing services for the practice by working as part of the practice nursing team

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols

Provide general and specific health screening to the practice patients (within agreed protocols) with referral to the general practitioners as necessary

To give appropriate health promotion advice to patients in whom risk factors are identified, according to agreed practice protocols and local and national targets

Advise patients on general health care and minor ailments with referral to general practitioners as necessary

To participate in health promotion for patients with chronic diseases such as diabetes, asthma and CHD, according to agreed practice protocols

To prepare/assist/facilitate vaccinations and immunisations and travel programmes

To undertake cytology screening

To undertake the collection of pathological specimens including intravenous blood samples, swabs, etc., and perform any investigatory procedures requested by the GPs following appropriate training

To participate in a family planning service to patients

To provide appropriate regular follow up for patients when necessary

To undetake Cannulation training and participate in the administration of infusions within our infusion suite.
