Job summary

An exciting opportunity has arisen for a passionate and motivated Social Prescriber to join our well-established and supportive Wellbeing Team working 20 - 25hrs per week. You must be available to work Wednesdays and Fridays.

This is a rewarding, patient-facing (non-clinical) role where you will work as part of a dynamic multidisciplinary team, supporting patients of all ages to improve their health and wellbeing.

As a Social Prescriber, you will:

  • Provide personalised support to patients to address practical, social and emotional needs impacting their health

  • Signpost and connect individuals to local activities, community groups and support services

  • Work one-to-one with patients within the surgery, in the community, or in their own homes

  • Facilitate and support community groups where required

  • Empower patients to build confidence, resilience and independence

Main duties of the job

  • Act as the point of contact for social prescribing referrals, communicating with patients by phone or in person.
  • Support patients and carers to access services or activities relevant to their needs
  • Work in partnership with all local agencies to raise awareness of social prescribing
  • Give patients a voice, ensuring they influence operational and strategic direction of the service
  • Be a key point of contact for all non-clinical referrals within the wellbeing team
  • Work sensitively and confidentially with people, their families, and carers
  • Support Health Champion volunteers.
  • Attend team meetings, 1:1 reviews and wider practice meetings as required
