An exciting opportunity has arisen for a Social Prescriber to join our well established Wellbeing Team within the practice. Working 25-30hrs per week.
The Social Prescriber will work as part of a multidisciplinary team in a patient facing (non clinical) role. Providing a service to patients of all ages. Signposting patients to activities, groups and services in their community to meet the practical, social and emotional needs that can affect their health and wellbeing.
They will work with individual patients in the surgery, within the community or patients own home. As well as facilitating community groups when required.
This role is part of the Network Contract Directed Enhanced Service (DES) - Additional Roles Reimbursement Scheme for Primary Care Networks.
Interviews for this post will take place on 2nd July 2025.
Main duties of the job
- Act as the point of contact for social prescribing referrals, communicating with patients by phone or in person.
- Support patients and carers to access services or activities relevant to their needs
- Work as part of a Multidisciplinary Team (MDT), attending local authority and VCSE meetings to discuss complex care cases, supporting colleagues to deliver excellent care at the appropriate time
- Connect with patients to improve health and wellbeing, introducing or reconnecting patients to community groups and statutory services
- Work in partnership with all local agencies to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care. Be proactive in encouraging self-referrals and connecting with all local communities.
- Give patients a voice, ensuring they influence operational and strategic direction of the service
- Be a key point of contact for all non-clinical referrals within the wellbeing team
- Work sensitively and confidentially with people, their families, and carers
- Support the Wellbeing Team to manage and organise the practices, Health Champion volunteers.
- Attend team meetings, 1:1 reviews and wider practice meetings as required
