An
exciting opportunity has arisen for an experienced Advanced Nurse Practitioner to
join our highly respected and friendly practice. Our well-established practice
has a committed team and excellent performance against local and national
requirements. Gillingham Medical Practice is a two-site PMS practice with six
GP Partners, providing high quality primary healthcare to a geographically
extensive rural population of over 12,500 patients.
An autonomous
practitioner working independently and in conjunction with other health care
professionals within their professional boundaries to provide care for patients
at Gillingham medical Practice. An experienced nurse that will provide care for
the presenting patient from initial history taking, clinical assessment,
diagnosis, treatment, including prescribing and evaluation of their care.
Main duties of the job
The
right candidate will be confident in working independently to support GP
surgeries. The candidate will be able to assess, examine, diagnose and treat
patients or escalate them to a GP appointment, as appropriate. The ability to
undertake some home visits is desirable. The candidate will be able to manage
varying workloads and be confident to adapt to the changing demands of GP
Practice.
Responsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness seeing and advising people in respect of their continuing medical and nursing needs. These patients will have booked appointments at ten-minute intervals, be a phone call or home visit. There may be a requirement to see extra or emergency patients. The practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.

Diagnosing and treating patients presenting with minor illness.
Ensure clinical practice is safe and
effective and remains within boundaries of competence and to acknowledge
limitations.
- Provide
first point of contact within the practice for patients presenting with
undifferentiated, undiagnosed problems, making use of advanced skills in
history taking, physical examination, problem solving and clinical
decision making to establish a diagnosis and management plan.
- Instigate
necessary invasive and non-invasive diagnostic tests and investigations
and interpret findings and action within own competence.
- As a non-medical
prescriber, prescribe and review medication for therapeutic effectiveness,
in a safe, effective and appropriate manner as defined by current
legislative frameworks and local guidelines, and within scope of practice.
- Provide
safe, evidence-based cost effective individualised care to the practice
population.
- Refer
patients directly to other services/agencies as appropriate.
- Work
with the multi-disciplinary team within the practice and contribute to
practice targets both local and national with disease management agenda. E.g.
QoF.
- To
undertake scheduled home visits for patients unable to attend the surgery
as agreed with the Duty GP and within scope of practice.
- To
provide regular reviews for residents within a local care home, including:
- Medication reviews within area of competence,
- Monitor medication requests for compliance
- Annual reviews for chronic ailments
- Update QoF as appropriate for each individual
- Refer residents to secondary care or other members of the multi-disciplinary
team as needed
- A point of contact for advice in the first instance for care home
staff. Be liaison between residents own GP and care homes
- Update admission avoidance care plans where appropriate
- Provide
patients access directly to NP via online booking (still in trial period),
booked slots via reception staff and offer advice and guidance via booked
telephone calls.
- Triage
and file OOH discharge notes and A&E letters on a daily basis,
ensuring follow up if needed, and ensure patient own GP aware of issues
arising from these contacts, including ensuring appropriate read code
added if indicated, GP is made aware of safeguarding issues in a timely
manner.
- Support
other healthcare professionals within the practice and offer advice and
guidance within area of competence.
- Dementia and Learning disability annual reviews
Administration

- Participate in the administrative and
professional responsibilities of the practice team
- Ensure accurate and legible notes of all
consultations and treatments are recorded in the patients notes
- Ensure the clinical computer system is kept
up to date with accurate details recorded and amended
- Dealing appropriately with results and
clinical letters for patients under your care.
- Ensure appropriate items of service claims
are made accurately, reporting any problems to the practice administrator
- Ensure accurate completion of all necessary
documentation associated with patient health care and registration with
the practic
- Ensure collection and maintenance of
statistical information required for regular and ad hoc reports and audit
