About the UEC Service

The UEC Hub operational team, oversees the delivery and function of the UEC Hub single point of access (SPoA) on behalf of the Cambridgeshire and Peterborough ICS. This will include the provision and management of the system UEC Hub Trusted Assessors.

The service will operate 7 days per week from 8am 8pm; however, these hours are subject to review in collaboration with the ICS and provider. The Hub will be accessible to Healthcare Professionals and will accept urgent/same day response referrals for patients who are registered with a Cambridgeshire and Peterborough GP Practice, and aged 18 or over.

The UEC Hub is a partnership between community, ambulance, primary care, acute services and social care that brings together a multidisciplinary team (MDT), that can prevent avoidable ambulance dispatches and conveyances, emergency department (ED) attendances and hospital admissions. Ensures timely transfer of appropriate patients from ambulance stack into alternative receiving services by improving interoperability, for example implementing interoperability toolkit messaging as a minimum

Main duties of the job

Deal with external and internal queries. This may include providing non-clinical information or advice to patients and external stakeholders.

Work with Grantas senior management team, the UEC Hub Administration Lead, Clinical Trusted Assessors and wider MDT to ensure that agreed Standard Operating Procedures (SOPs) are followed, and updated as necessary, to ensure a universal way of working throughout the team.

Provide extensive administrative support to the team to ensure the smooth running of the service

To actively participate in the development of improvements for the service. Incorporate new practices to take account of new technology, demand/capacity tools.

Support other members of the team as required to ensure the smooth running of the department.

Monitor the hub inbox and be the first point of contact for non-clinical queries.

Update and maintain a patient tracker where cases are being managed by the hub.

Prepare and draft documents as requested, maintaining the highest level of professional standards.

Support the team in gathering any data for audit research purposes.

Work with UEC Hub team and wider system partners to ensure support of completion of any workload and tasks necessary

Identify issues and actions required using initiative and understanding of own authority to resolve any issues and/or escalate when necessary

The Urgent Emergency Care (UEC) Hub Administrator will provide pivotal administration support to the delivery and coordination of our hub which is an integral part of the out of hospital care model designed to ensure that patients receive appropriate care and treatment as close to home as possible.

Operating 365 days per year, the UEC Hub receives referrals between 8am to 8pm seven days a week from a range of health and care professionals which require triages and assessment to determine the right care at the right time for the patient to ensure they can remain safely at home. Working as part of a team, the post holder will work a shift pattern across 7 days a week. This roles priorities will include:

o Consistent provision of administration support to the UEC Hub team

o Ensuring a high standard of administrative service is consistently delivered by undertaking all required tasks and activities on a daily basis.

o Utilising the required technology/ system(s) that are introduced and support other team members to do so to deliver an efficient administrative service.

o Ensuring organisation, confidentiality, and sensitivity throughout all assignments

o Supporting the team by ensuring a clear and universal way of working in a team, effectively ensuring escalation of any risks and/or issues as appropriate
