Job summary
The Pinn medical centre have had pharmacists within our primary care
team for more than 30 years. We have a large and friendly pharmacy team, who
work together to ensure that we provide optimal care to all of our patients. We
are looking for candidates who can undertake medication reviews, repeat
prescribing, provide leadership on patient centred medicine optimisation and
quality improvement, whilst contributing to the quality and outcomes framework.
Depending on the skillset of the candidate, we would welcome experience in
chronic disease management, such as diabetes, heart failure, COPD, or other
areas. We are keen to upskill any candidates, and work with them to develop
innovative new pathways that can help those patients who chronic disease to
take better control of their outcomes.
The post holder will work as part of a multi-disciplinary team,
including a patient facing role. Over time the post holder will take
responsibility for areas of chronic disease management within the practices and
undertake clinical medication reviews to proactively manage patients with
complex polypharmacy. We will support the successful candidates continued
professional development.
Main duties of the job

- Patient facing Long-term condition reviews - Take responsibility for the care management of patients with chronic diseases where medicine optimisation is required.
- Telephone and Patient facing clinical medication reviews - Undertake clinical medication reviews and make appropriate recommendations for the team on prescribing and monitoring.
- Patient facing medication reviews for housebound patients - Undertake clinical medication reviews with patients and make recommendations for the clinical team on prescribing and monitoring. Work with carers and care home staff to improve safety of medicines ordering and administration.
- Management of repeat prescribing and develop protocols across network practices for re-authorisation process
- To have a leadership role in supporting further integration of general practice with the wider health care teams to help improve patient outcomes, ensure better access to healthcare. The role has the potential to significantly improve the quality of care and safety for patients.
- Take a central role in the clinical aspects of shared care protocols, audit, clinical research with medicines, liaise with specialist pharmacists, community pharmacists and drug monitoring including anticoagulation and DMARDS.

- Provide specialist expertise in the use of medicines whilst addressing both the public health and social care needs of patients at PMC and help in tackling inequalities.
- To have a leadership role in supporting further integration of general practice with the wider health care teams (community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
