Job summary

We are looking for highly motivated Advanced Nurse Practitioner to join our dynamic and friendly team to work in state-of-the art GP Led Health Centre, rated "Outstanding " by CQC, in Pinner which is open 7 days a week. We are an innovative and forward thinking EMIS practice committed to delivering the highest standard of patient care.

Main duties of the job

  • To work as an advanced nurse practitioner, seeking advice where necessary from our supportive clinical team, to assess, diagnose and treat the conditions of patients attending the primary care
  • Provide professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence
  • To attend and contribute to education and training within the multidisciplinary practice team

Job responsibilities

PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES

  • The Advanced Nurse Practitioner works autonomously, and seeks advice where necessary, from our supportive on-site clinical team
  • Undertakes Clinical Nursing Practice at an advanced level using clinical knowledge and skills to deliver holistic care to people accessing primary health care services
  • Assesses, diagnoses and treats patients attending primary health care services with a range of acute, non-acute and chronic medical conditions, seeking advice from the multidisciplinary clinical team where necessary

Interprets a range of diagnostic tests and routine clinical procedures, seeking help from our supportive senior clinical team where necessary

Works within professional guidelines and codes of conduct, using clinical judgement

Works within all relevant practice policies and procedural guidelines e.g. infection control, chaperoning, risk management.

Independent prescriber desirable

Administration and professional responsibilities

  • Participate in the administrative and professional responsibilities of the Practice team
  • Ensure the clinical computer system is kept up to date with accurate details recorded and amended
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
  • Attend and participate in Practice, nursing and educational meetings as required

Training and personal development

  • Training requirements will be monitored by yearly appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the Practice.
  • Participate in the education and training of students of all disciplines and the introduction of all members of the Practice staff where appropriate
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development
  • Develop and maintain a Personal Learning Plan

Liaison

  • As well as the nursing team there is a need to work closely with reception, and administrative staff to ensure the smooth running of the Practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team.
  • There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care
