Job summary

We are seeking an experienced and motivated qualified Practice Nurse to join our clinical team. You will play a key role in delivering high standards of patient care, chronic disease management, and preventative health services.

Main duties of the job

  • Deliver high-quality nursing care within a GP practice setting
  • Chronic disease management (e.g. diabetes, asthma, COPD, hypertension)
  • Cervical cytology screening
  • Immunisations and vaccinations (including childhood and travel vaccines)
  • Wound care and dressings
  • Health promotion and screening
  • Supporting QOF and local enhanced services
  • Infection Control lead
  • Maintaining accurate clinical records in line with NHS and practice policies

Job responsibilities

  • Dressings
  • Ear syringing
  • Venepuncture
  • Providing family planning services
  • Teaching self-examination (breast and testicles)
  • Chronic disease management programmes e.g. diabetes, asthma, hypertension and others
  • Cervical smears
  • Routine immunisation (Baby, Child and Adult)
  • Obtaining specimens to be sent to the laboratory
  • Observation of patients following procedures in surgery
  • Taking and recording blood pressures
  • Giving advice on healthy living
  • Smoking cessation advice
  • Screening new patients
  • Visiting patients at home as requested by GP for vaccination and other clinical procedures according to protocols. As all visits are in the local area, no transport is provided and they should be done on foot.
  • Assessing the needs of the elderly in the surgery or at their homes if patients are unable to journey to the practice (including annual over 75 checks).
