In line with the NHS Cancer Plan Colchester Medical Practice
Primary Care Network (CMP PCN) aims to facilitate the diagnosis of more cancers
at an earlier stage for patients and to provide genuinely personalised care and
support to all patients with a cancer diagnosis.
We are recruiting for a PCN Cancer Care Nurse to drive this
agenda forward, as part of a unique pilot role. We expect that half or more of your time will
be spent carrying out 3 month and 12 month Cancer Care Reviews with the
remaining time spent between increasing uptake of screening and testing such as
FIT and Cervical Smears and working to develop a more integrated patient
friendly pathway between primary care, secondary care and the voluntary sector.
Operating from the four CMP sites, the successful applicant
will be an experienced NMC Registered Nurse ideally with experience in Primary
Care or Community Care and with clinical experience of cancer care.
S(he) will have high levels of initiative and self-organisation
combined with the confidence to interact to good effect with GPs and other
clinical colleagues.
This is a full-time
post which is remunerated at £43,679.04 per annum and for an initial contract
period of 18 months. 6 weeks annual leave plus bank holidays, NHS Pension. Clinical
support / supervision will be provided. CMP is a large, diverse and friendly team. Closing date for
applications 30 09 2023 start date by agreement.
Main duties of the job
Scope of
the role
To carry out all 3 month and 12 month cancer
care reviews with CMP Patients ensuring that personalised care is provided,
that patients understand what support is available in their community and that
they have the information to begin supported self -management.
To support the 2WW suspected cancer pathway
referrals into secondary care, by ensuring all pre referral tests are
completed. Working within an agreed framework that includes safety netting and
returning to the referrer.
To work for CMP to scope the community services
available that support early identification of cancer. This role will
link into local communities and Voluntary sector organisations within the local
area to support education.
Lead on the development of a Make Every Contact
Count through embedding a health promotion pathway for those patients
who have been referred on a cancer pathway and are discharged following
secondary assessment. Those patients who undertake risk taking behaviour such
as smoking or excess alcohol may be our future cancer patients. To lead improvements in the quality of Cancer
Care Reviews completed in primary care post diagnosis. To support the development of innovations across
all aspects of the cancer pathway.

About us
Colchester Medical Practice came about as a result of the mergers of four different GP Practices who came together in recent years to foster GP resilience and alliance working.
There are 13 GP Partners of whom three at any given point serve as Executive Partners and are engaged in the active day to day running of the practice and PCN. One of the members of the Executive Group is also Clinical Director of the PCN.
We are extremely fortunate to have a significant number of experienced and skilled individuals working at all levels across our organisation. Whilst recruitment and retention are never easy in primary care CMP is fortunate in having retained a core of excellent team members.
As a practice, CMP is big enough to have a high degree of resilience and as a PCN, CMP is small enough to encourage genuine innovation in preventive and anticipatory care.
