Job summary

Colchester Medical Practice Primary Care Network is a unique single practice PCN spanning 4 sites and a well being hub across central locations in Colchester. We are looking for full time OR part time ANP(s) who (ideally) have GP experience to join our large and friendly multi-disciplinary team operating a same day face to face access system. You will be well supported and encouraged to grow in your professional practice by the partners and your established ANP colleagues, with regular training events on offer. Main duties of the job

We expect that you will be working mostly, if not all the time at one specific site, which will allow you to from relationships with other staff and clinicians, becoming integrated into the overall team.

You will be expected to:

Independently deliver care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

Deliver safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the General Practice.

To take on solve and reduce GP workload directly within the scope of your clinical practice.

About us

Colchester Medical Practice came about as a result of the mergers of four different GP Practices who came together in recent years to foster GP resilience and alliance working.

There are 13 GP Partners of whom three at any given point serve as Executive Partners and are engaged in the active day to day running of the practice and PCN. One of the members of the Executive Group is also Clinical Director of the PCN.

We are extremely fortunate to have a significant number of experienced and skilled individuals working at all levels across our organisation. Whilst recruitment and retention are never easy in primary care CMP is fortunate in having retained a core of excellent team members.

As a practice, CMP is big enough to have a high degree of resilience and as a PCN, CMP is small enough to encourage genuine innovation in preventive and anticipatory care.

The Advanced Nurse Practitioner will work across different sites as part of a thriving ANP Team consulting face to face, by telephone and online where necessary. You will be able to see and treat undifferentiated patient presentations, formulate a treatment plan and prescribe safely, appropriately and according to formulary and local guidelines.

Primary responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

b. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

c. Make clinical referrals using agreed local pathways

d. Make requests for diagnostic imaging in line with local guidelines

e. Carry out home visits to patients where clinically appropriate

f. Participate as directed in all Population Health Management initiatives and work towards agreed Health Inequality targets in partnership with other agencies and stakeholders through the Primary Care Network

g. Develop, implement and embed health promotion and wellbeing programmes, implement and evaluate individual specialised treatment plans for chronic disease patients and identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health

h. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

i. Request pathology services as necessary, process and interpret pathology and other test results as required

j. Maintain accurate clinical records in conjunction with extant legislation

k. Support the team in dealing with clinical emergencies

l. Recognise, assess and refer patients presenting with mental health needs

m. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

n. Contribute to practice targets (QOF etc.), complying with local and regional guidance

o. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

p. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual

q. Support the clinical team with all safeguarding matters in accordance with local and national policies

r. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

s. Deliver opportunistic health promotion where appropriate

Secondary responsibilities

In addition to the primary responsibilities, the Advanced Nurse Practitioner may be requested to:

a. Take part in clinical audit

b. Acting as a mentor to colleagues, students and newly qualified staff

c. Participate in local initiatives to enhance service delivery and patient care

d. Support and participate in shared learning within the organisation

e. Develop an area of specialist interest, taking the lead within the organisation

f. Continually review clinical practices, responding to national policies and initiatives where appropriate

g. Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA)

h. Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required

i. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation
