forefront of our ethos. We are looking for a Care Navigator to join our bubbly
Care Navigation team, providing guidance and care to our patients.
Care
Navigation within a GP surgery is varied, interesting, sometimes busy,
frontline work. If you have great communication, organisation, and IT skills,
can remain calm under pressure and prioritise your work efficiently, then this
could be the role for you.
Previous
experience is desirable, but full training will be provided for the right
candidate.
Working
pattern will be 10.5 hours per week, to be discussed at interview. The
successful applicant will be expected to cover a reasonable amount of staff
leave and sickness, sometimes at short notice.
PAY
- £11.50 per hour.

Main duties of the job
Care
Navigation requires daily face to face, telephone and online communication with
patients, to signpost, arrange appointments and follow-up care.
You
will be trained and responsible for directing patients to the most appropriate
service or healthcare professional, in a compassionate and timely manner.
You
will also be expected to assist the team with administration tasks and build
relationships with partner services.
