Job summary

Oakfields Health Group is looking to recruit a practice Nurse.

This is a permanent contract - 30 hours per week.

The successful candidate will be required to work across both practice sites, supporting an experienced, skilled and highly motivated team.

The salary for this role is commensurate with experience.

Main duties of the job

About you:

  • Highly motivated and enthusiastic
  • Minimum 2 years' experience in General Practice essential
  • Must be able to take the lead in COPD & Asthma
  • Experience in Diabetes, Family Planning, Cytology & Childhood Immunisations
  • Good knowledge of Chronic Disease Management and QOF

Job Title: Practice Nurse

Hours: 30 hours per week

Reports to: Senior GP Partner

Location: Oakfields Health Centre / Dipton Surgery

2. JOB PURPOSE

To undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with the GPs, Nurse Practitioners and other practice nurses or independently, in line with competence and need.

To maintain or improve standards of care and be actively involved in identifying practice population needs.

To demonstrate and apply knowledge related to the Quality and Outcomes Framework (QOF).

To provide therapeutic interventions within a broader health promotion/public health strategy and enable patients to take action to meet their priorities for health.

To help provide education and training regarding chronic clinical conditions as required.

To provide specialist advice to patients and relatives in relation to COPD, Asthma and, where qualified, Diabetes chronic disease conditions.

To undertake any further training required to ensure appropriate role development.

To practice within the NMC Code of Professional Conduct and maintain accurate records.

3. KEY RESULT AREAS

Professional Responsibilities

Assist practice GPs/NP to deliver effective and holistic management of patients with chronic diseases as required by the Quality and Outcomes framework.

Contribute to the development of policies / protocols / guidelines within the practice.

Aid the practice team with implementation of new initiatives in the delivery of chronic disease management/enhanced services.

Formulate reports as required for the business manager and or nurse practitioner.

Support the implementation of evidence-based guidelines in conjunction with the practice clinical team.

Encourage the development of integrated services within Primary Care to include the practice based teams and multidisciplinary working with external providers.

Promote the continuing professional development and skills of both yourself and other nurses within the practice.

Leadership

Provide professional advice and support to the practice nursing teams.

Offer guidance and support to the practice nursing teams to maintain/improve standards of care.

Participate in chronic disease management within the practice & provide practical assistance and professional advice to achieve the highest standards of care.

Provide clinical input where required alongside other practice nurses, particularly newly appointed nurses, to assist in skill development and sharing of best practice.

Delegate to others, with appropriate skills, knowledge and competence.

Maintain nursing practice in line with relevant evidence based research.

Act as a resource to the general public, patients within the health care system, their families, carers and external healthcare professionals.

Clinical

Work alongside other practice staff to help develop their knowledge and skills.

Support the practice in achieving 100% QOF and enhanced service targets.

Undertake opportunistic screening when clinically required/indicated.

Perform venepuncture and electrocardiography where required.

Undertake wound care/ management/dressing techniques.

Carry out ear examination and syringing.

Undertake family planning assessments, provide advice and refer patients appropriately.

Perform cervical screening and promote breast awareness.

Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs.

Undertake accredited training/updates in chronic disease management.

Contribute to the maintenance of the practice disease registers.

Provide advice on lifestyle issues, assist and support patients in changing their behaviours and refer appropriately.

Support the adult and childhood immunisation service working to Patient Group Directions.

Deliver travel healthcare, including vaccinations and advice for patients.

Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies and demonstrate awareness of local referral procedures.

Support the implementation and effective use of the child safe trigger tool.

Apply infection control measures when collecting and handling laboratory specimens, disposing of waste materials, dealing with blood and body fluid spillages and report and treat sharps injuries according to protocol.

Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques.

Research, Audit and Quality

Support the practice audit strategy, action plan and evaluation.

Contribute to the provision of equitable, high quality services for patients in the locality.

Help support the integration across primary, secondary and tertiary care services.

Maintain an interest in national research initiatives (NICE) and disseminate appropriate information to health professionals as required.

Ensure all care is evidence based using up to date validated literature.

Education and Training

Provide professional advice/support to healthcare professionals within the Primary health care team.

Following identified training needs, participate in training, facilitation and clinical supervision of other practice nurses.

Provide mentorship and guidance for junior staff.

Participate in partnership programmes, raising awareness, detection and prevention of chronic diseases.

Maintain a high level of expertise in specific chronic disease management, specifically Diabetes, COPD & Asthma and seek to develop new and improved skills.

Advise members of the public on the concepts of health promotion, education, prevention and protection.

Seek out new knowledge of nursing and health by reading, enquiring and partaking in internal and external continuous education programmes.

Take responsibility for personal development and education including in-house training & monthly PLT.

Actively participate in multidisciplinary education and training.

Direct, supervise and help mentor junior nursing staff in the clinical setting.

Monitor and maintain consistency of professional standards and take appropriate action if standards are not met.
