You will lead the nursing team and will undertake all aspects of the practice nurse role including cervical screening, baby immunisations and chronic disease management
Main duties of the job
The Practice Nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policies and procedures
Clinical tasks include:
- General practice nurse duties
- Cervical Cytology
- Wound care and dressings
- Chronic disease management (COPD & Asthma)
- Routine and child immunisations and vaccines
- Lead on infection control
- Have good understanding and knowledge of QOF
- Knowledge and experience of working within the framework of locally commissioned services and long term conditions
- Must have GP experience

We are a PMS training practice and have over 9,300 patients & with CQC rating good
The practice's aim is to provide our patients with an excellent standard of patient-centred care, with clinicians and patients jointly involved in all aspects of planning and delivering care to meet patients' health and well-being needs
You will work with colleagues including a team of GPs, GP trainees, clinical pharmacist, MSK practitioner, practice nurse, physician associate, social prescriber, healthcare assistant and administrative staff and a wide variety of other health professionals

The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policies and procedures.
Duties and Responsibilities:
Patient Care:
- Management of chronic diseases, including diabetes, asthma, and hypertension
- Administration of cervical cytology, routine vaccinations & injections and baby immunisations.
- Conducting urine testing, blood pressure monitoring, ECGs, and other diagnostic tests
- Wound care, including the removal of sutures
- Providing advice on contraception, safe sex, and self-management of diseases
- Managing minor injuries, burns, and other acute conditions
- Chaperoning during clinical procedures as required
- General nursing duties
Record Keeping and Administration:
- Maintain accurate and contemporaneous records of all patient encounters
- Label and document patient samples accurately for analysis
- Work within practice protocols for safe data management and patient record coding
Maintaining a Safe Clinical Environment:
- Comply with health and safety policies to minimise risks
- Report hazards and adhere to infection control policies, including proper handling of clinical waste and sharps
- Prepare clinical areas, manage stock, and ensure equipment is maintained and calibrated
Supplies Management:
- Monitor and order clinical supplies and vaccines while maintaining accurate stock records
- Ensure compliance with cold chain protocols and maintain updated health information displays
Quality and Governance:
- Deliver patient care to the highest standards, monitor service quality, and contribute to clinical audits
- Participate in complaints reviews, significant event analysis, and shared learning sessions
- Provide and receive constructive feedback to support team performance
Communication:
- Clearly outline the scope of the PN role to patients
- Modify communication methods to address barriers and ensure patient understanding
- Actively participate in practice and team meetings
Personal Development:
