mental health practitioner to join our team at a well-established general
practice. We are delighted to be able to expand our already well complemented practice team and increase the number of Mental Health Practitioners in the team through participation in the NHSE PCN Pilot Programme as part of Meridian Health Group Primary Care Network (PCN). mental health challenges.
Main duties of the job
Responsibilities:- Conduct comprehensive mental health assessments and develop personalized treatment plans for patients.- Direct to appropriate services to provide evidence-based interventions and therapy to individuals, couples, and families dealing with various mental health issues.- Collaborate with the general practice team, including doctors, nurses, and allied health professionals, to ensure integrated care for patients.- Offer primary care level of crisis intervention and support to patients in need of immediate assistance.- Educate patients and their families about mental health conditions, treatment options, and self-care strategies.- Maintain accurate and up-to-date clinical documentation and patient records.- Participate in case conferences and multidisciplinary team meetings to discuss patient progress and coordinate care.

The Roxton Practice is a large dispensing partnership delivering
services at three sites; Pilgrim Primary Care Centre, Immingham; Weelsby View
Health Centre, Grimsby; and the Health Centre, Keelby. Services are also
provided in the community in North East Lincolnshire. We have over 34,000
registered patients and more than 170 staff with a strong patient focused

The Roxton Practice is part of the Meridian Health Group
Primary Care Network (PCN) covering a population of 40,000 patients. The PCN
works in collaboration with other local practices and specialist community and
acute providers to develop and improve services for our patients. The PCN
provides extended access, a community frailty service, care home services and
has changed the delivery model of outpatient services via its work with the
local acute provider in developing the Connected Health Network.

- -Experience in prescribing psychotropic medication and be an independent prescriber or working towards this
- - Valid license or registration as a mental health practitioner in the relevant jurisdiction.
- - Proven experience working as a mental health practitioner, preferably in a general practice or primary care setting.
- - Strong knowledge of evidence-based therapeutic interventions for a range of mental health conditions.
- - Excellent communication and interpersonal skills, with the ability to establish rapport and build trust with patients.
- - Ability to work effectively as part of a multidisciplinary team, collaborating with healthcare professionals from various backgrounds.
- - Empathy, compassion, and a non-judgmental approach to patient care.
- - Strong organisational skills and the ability to manage a caseload effectively.
- - Commitment to ongoing professional development and staying up-to-date with current research and best practices in mental health.
