ABOUT THE ROLE

We are looking for an experienced Advanced Clinical Practitioner, independent prescriber, with a digital badge in advanced clinical practice to join in our care home team.

Main duties of the job

As a team of clinicians we provide regular ward rounds in care homes, assessing patients for acute illness, as well as providing proactive care such as medication reviews.

The team is a multi-disciplined team with paramedic, nurse, GP and care coordinator backgrounds, based at Keyworth, and covering Rushcliffe PCN. We have a very experienced GP partner clinical lead, plus support from the NUH community geriatrician service and aligned GP practice for each care home.

We are looking for an ACP who has a passion for frailty management and being part of a dynamic team.

About us

This job requires the postholder to travel and have face to face consultations with residents in care homes, undertake remote consultations and discussions, liaise with GP practices, and the aligned GPs, as well as with other health and social care providers, residents and relatives.

PURPOSE OF THE ROLE

The purpose of this role is to provide clinical consultations as part of a ward round in a Rushcliffe PCN older person care home.

This role will utilise your advanced clinical skills as autonomous practitioner. Providing clinical assessments of residents presenting with acute and non-acute medical needs, undifferentiated and long-term condition presentations. Consultations include assessment, examination, investigation, diagnosis, and treatment plans.

Support the delivery of the National Enhanced Healthcare in Care Homes, Direct Enhanced Service Framework (DES).

Main duties of the Role

Our service operates Monday to Friday, with no late shift, nights, weekend or bank holiday requirement.

Medications management is a key part of patient assessments, and are completed as part of the clinical assessment or proactively including planned medication reviews.

As part of the patient assessments, ReSPECT planning is part of the wider patient consultation and discussion which may include relatives as part of the decision making.

As part of the team, you will support the delivery of COVID, & flu vaccines as part of the National vaccination programme.

As a senior clinician, you will provide clinical leadership and expert professional advice to patients, relatives, carers, and colleagues to support the delivery of high-quality clinical care, and this will, at times involve teaching, and education of Care Home or other staff.

As an ACP, you will be supported by Rushcliffe PCN GP clinical lead as ongoing CPD development. As well as wider clinical support working with the Rushcliffe GPs and NUH community geriatricians.

This job requires the postholder to travel and have face to face consultations with residents in care homes, but will also undertake some remote consultations and discussions.

ADMINISTRATION

Ensure consultations are documented accurately, clearly, and contemporaneously, in a timely manner.

Work with clinical ledgers, ensuring admin is up to date.

Update patient records in accordance with PCN policies and procedures meeting the requirements of professional bodies NMC/HCPC. You will support delivery of the metrics required within the Enhanced Healthcare in Care Homes are met.

Ensure continuing education, training and development is undertaken to meet clinical governance guidelines for Continuing Professional Development and a Personal Development Plan.

Actively participate in clinical notes and prescribing audits, along with clinical supervision as part of the ongoing CPD and governance compliance requirements.

Maintain professional registration with the NMC/HCPC as required to maintain the healthcare professional registration.

Provide evidence of a digital badge in Advanced Clinical Practice or hold the ACP qualification and actively progressing portfolio completion.

Undertake a fair share of the overall administration associated with clinical practice within the care home team, including answering the phone, supporting ledger management etc.

To participate in staff meetings, 1-2-1 meetings and the appraisal scheme and undertake training appropriate to the development of the job role and in line with the PCNs statutory obligations.

To work to the agreed standards, policies and procedures and protocols of the PCN and be fully conversant with emergency procedures.

To reflect on own practice individually and/or through regular clinical supervision.
