HEALTH CARE ASSISTANT JOB DESCRIPTION
JOB TITLE: Healthcare Assistant
BASE: East Leake Health Centre, but will also be required to work at
Church House Surgery, Ruddington or
Sutton Bonington Branch or

LINE MANAGER: Surgery Manager
REPORTING TO: GPs and Practice Nurses
JOB PURPOSE
To provide Health Care Assistant services by working as part of the Primary Health Care Team, providing care and support to patients and carers in both clinic and home environments.
To act as receptionist by providing a point of contact for patients and acting as a focal point of communication and provide support for the GPs and Clinical Staff.
MAIN DUTIES
1. Health Care Assistant/Clinical Duties
To undertake technical and clinical procedures such as venepuncture, urine testing, taking blood pressures or doing ECGs and other clinical tasks as agreed. This may be ad hoc as requested by GPs or Practice Nurses or may be in dedicated clinic time.
Procedures will be carried out in line with practice protocols after satisfactory completion of training and testing of competency. Current list of required competencies attached.
Prepare and maintain clinical environments according to health and safety and infection control policies. Ensure all equipment is kept clean and ready for use with sufficient in-date supplies in each consulting room.
Contribute to the ordering, distribution and maintenance of equipment ensuring stock levels are maintained at appropriate levels.
Carry out domiciliary visits to carry out venepuncture and health checks as required.
Assist in the delivery of programmes of care to address peoples health and well being needs.
Support the GP & nursing team by undertaking duties to help ensure the smooth running of the team.
Maintain accurate, timely and legible records of patient care, adding and retrieving information as required to and from computer records.
Feedback and report on the condition and welfare of patients to the appropriate clinician.
Assist in data collection for audit and recall purposes and participate in research and audit activity as required.
Assist in the proactive management of chronic diseases including monitoring QOF targets and ensuring patients receive appropriate follow up care.

2. Management of Appointment System and Reception
To book and manage routine and urgent appointments ensuring that sufficient information is recorded.
To book and notify patients of appointments by post and telephone for extra clinical sessions, maintaining electronic waiting lists and follow up procedures.
To act as receptionist for patients and visitors to the Surgery, to deal with face to face and telephone queries and passing of messages and other information. To respond to all queries and requests for assistance from patients and other visitors. Ensure appropriate response according to the nature and urgency of a message.
Enter requests for home visits onto the clinical system stating time received and including all relevant information and where necessary refer to the duty doctor.
Advise patients of relevant charges for private services, accept payment and issue receipts for same.
Action requests for ambulance transport and record details.
Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, ensure procedures and paperwork are completed.
Ensure total familiarity with all systems in effect, including regular and incidental variations.
3. Telephone System
Receive and make calls as required. Divert calls and take messages as appropriate.
If working reception ensure that the system is operational at the beginning of each day and switched over to the out of hours service at the end of the day.
4. General
Assist in the provision and maintenance of up to date information for the public, particularly notices, posters and leaflets, in a tidy and accessible form.
To be flexible in order to cover Bank Holiday and Saturday rotas plus staff absences due to sickness and holidays.
To undertake other duties considered appropriate to the post.
OTHER REQUIREMENTS, KNOWLEDGE AND SKILLS
An understanding, acceptance and adherence to the need for strict confidentiality.
Keep up to date with current CPR and other mandatory training.
Attend in-service training and other meetings as required, taking every reasonable opportunity to develop, maintain and improve clinical skills and competencies.
To undertake appropriate training as agreed from time to time with the practice manager, practice nurses or doctors.
An ability to use own judgement, resourcefulness, common sense and local knowledge to respond to patients' enquiries and requests.
Excellent Communication Skills.
