ABOUT THE ROLE

If you are an innovative person and would love the exciting challenge of working within a new and developing patient-focused service within the Rushcliffe Primary Care Network. This could be the job for you. Our service operates Monday to Friday, flexibly between the hours of 8am – 18.30pm with occasional weekend or bank holiday working.

This exciting new role will operate across Rushcliffe to support the delivery of a number of the Primary Network key priorities. The post holder will work across organisational boundaries, developing new ways of working to promote relationships between providers and statutory bodies.

Main duties of the job

The role involves patient searches and data gathering, contacting and organising patient appointments, supporting staff rotas, being involved in projects and NHS initiatives, supporting the organisation of the COVID vaccination service and much more.

The successful candidate will have excellent interpersonal and communication skills, and be organised, patient and empathetic.

You must be able to travel across the locality for service delivery. Post - PCN Care Coordinator

PURPOSE OF THE ROLE

The post holder will work closely with teams to assist in co-ordinating care utilising population health intelligence. The role will promote integrated working across Rushcliffe Primary care network (PCN), General Practice and existing statutory and voluntary health and social care services.

The role supports a variety of services including, but not exclusive to; mental health, social prescribing, pharmacy, respiratory, vaccination, care homes, extended hours and other services identified as a priority within the Primary care network (PCN).

MAIN DUTIES AND RESPONSIBILITIES

· Understand what a primary care network is, and how support is provided to patients as a result of improved collaboration of working between health and social care services.

· Utilise data to help identify and work with groups of patients to deliver personalised care.

· Record patient interventions / contacts on relevant electronic database systems within agreed time scales (e.g. SystmOne / Pinnacle).

· Contribute to report generation, analysis and production using electronic systems such as e-Healthscope.

· Support patients to book appointments, as part of the various projects, programmes and clinical initiatives.

· Help people to manage their needs through answering queries, making and managing appointments, and making sure that people have understandable written or verbal information to help them make choices about their care.

· Support or manage clinics as required – including staff rotas.

· Assist people to access self-management education courses, peer support or interventions that support them to take more control of their health and wellbeing.

· Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care staff.

· Support the coordination and delivery of mutli-disciplinary meetings within the PCN.

· Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN.

· Raise awareness within the PCN of shared decision-making and decision support tools.

· Identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations.

· Provide coordination of, support and participate in relevant internal and external working groups and provide project support where requested.

· Work with key people in the PCN to develop & support collective general practice projects including areas of federated working.

· Work closely and in partnership with the Social Prescribing Link Workers.

· Adhere to organizations policies and procedures, guided by occupational policies and procedures in primary and secondary care.

· Maintain up to date skills and knowledge and maintain awareness of professional issues.

· Works without supervision, plan own workload, and seek guidance as required from line manager and colleagues.

· The post holder should demonstrate good organization and time management skills.

· Use clinical systems for record keeping, audit and evaluation.

· Develop and implement data collection systems that will provide accurate and timely data.

· Administrative support for clinicians.

· Maintain confidentiality at all times.

LEARNING & DEVELOPMENT

Ensure continuing education, training and development is undertaken to meet role specific and governance guidelines for Continuing Professional Development.

Continue to develop soft skills and emotional intelligence in line with the requirements and culture and to uphold the values and culture of PartnersHealth.

Undertake ongoing training as required for the role of the care co-ordinator.
