Specialist Diabetes Nurse

Job summary

The post holder is responsible for ensuring the delivery of diabetes and care for the whole practice population. They will work independently and in conjunction with other members of the clinical team to deliver excellent primary care services within the practice. Clinically, the focus of the role is the delivery of evidence-based practice for patients with diabetes conditions and other related metabolic conditions, management and preventative nursing interventions to all patients.

As autonomous practitioners, the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making. In addition to this, they will provide leadership and work collaboratively with the management team to support any transformation of services needed to ensure the organization that offer innovative, effective, safe clinical services that raise standards of care and reduce health inequalities.

Main duties of the job

To ensure that the Practices diabetes care is delivered in a timely, professional manner whilst maintaining equity of access to a coordinated and seamless service.

To identify patient's diabetes care needs using a holistic approach and to assess and offer highly specialist advice in all aspects of complex diabetes management to all members of the multidisciplinary team.

Chronic disease management - assess, plan, implement and evaluate individual treatment plans for patient, including insulin and GLP-1/GIP initiation,

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols

  • To demonstrate a positive supportive approach to patients at diagnosis and/or patients with the development of diabetes related complications

Work with patients in order to support adherence to prescribed treatments

  • Establish and maintain highly effective communication skills with patients, carers/relatives, and professionals across health and social services. This may involve the giving of sensitive and complex information.

    On occasion the post holder will be required to visit patients in their own home e.g. to conduct chronic disease reviews.
