Job summary

The Digital & Transformation Lead is employed by Healthwest PCN to lead the digital and strategic development of the PCN to help address health inequalities and improve patient care.

Main duties of the job

The main duties of this role are to efficiently manage all areas of IT across the practices and PCN with the aim to reduce variability between practices (where appropriate) and facilitating the sharing of good practice; to review the PCN digital estate and lead improvement in digital maturity, including cybersecurity and resilience plans; to develop business cases for digital transformation work, leading on the development and implementation of these projects.

Job description

Job responsibilities

- Co-ordinating support and resolutions for IT related issues, with assistance from the PCN IT Team and the relevant external support teams.

- Acquirement and setup of new software or hardware required on site.

- Staff training on software and hardware (e.g. Windows, EMIS, Telephony, Remote Working).

- Auditing and analysis of practice data and statistics (e.g. SNOMED, CQRS, Immunisations, Appointment Reporting, Population Management).

- Development, deployment and training for new projects agreed by the PCNs GP IT leads.

- Management of EMIS resources protocols, templates, concepts, document templates, searches & reports.

- Coordinate and chair PCN IT Meeting

- Lead the IT Team and conduct annual appraisals

- Aim to reduce variability between practices (where appropriate) and facilitating the sharing of good practice

- Develop communication strategy (both internal and external) in conjunction with the practices and other system partners

- Explore new technology opportunities to improve patient care, safety and efficiency across the PCN

- Generate training materials for new IT projects or software

- Develop relationships with other practices, One Care, the GPCB and other system partners to create and share digital solutions to improve patient access and care

- Develop business cases for digital transformation work

- Have responsibility for leading and facilitating quality improvement with clinical support

- Foster relationships with the ICS population health management to identify areas of need and improvement for the PCN population

- Review and improve the PCNs digital maturity

- Design/explore business intelligence tools to monitor activity in the practices and provide reports on capacity and utilisation

- Collaborate with system partners at neighbourhood level to improve digital communication, share data, IT development and recording of clinical activity.
