Job summary
We seek to recruit an experienced Practice Nurse, ideally working 15 to 16 hours across 2 days.
Long-term condition management is an
essential part of the role using evidence-based practice for condition
management including that of our housebound patients.
Ensuring nursing services and
assessments are delivered effectively to the practice population, providing
appropriate care / treatment in conjunction with GPs and ANPs and in accordance
with national and local guidance as well as practice protocols.
Supporting Healthcare Assistants in their
development of practical skills and adherence to relevant protocols and guidelines.
Maintain clinical responsibilities
for infection prevention and control, health promotion, NHS health checks,
immunisations, cervical screening, adult and childhood immunisations, reviewing
and implementing clinical policies and procedures.
Main duties of the job
Assist GPs in the development and implementation of
processes for the effective management of patients with long-term conditions
Support the Infection Prevention Control (IPC)
lead and work to IPC Policies, assist in the implementation of audits, training
and support
Develop, implement and embed an effective care
management programme for the frail and housebound patients
Develop, implement and embed health promotion, wellbeing
programmes and screening campaigns
Identify, manage, educate and support patients
at risk of developing long-term conditions with a focus on prevention
Administer vaccinations and immunisations for adults
and children in accordance with PGDs
Review patient's long-term condition medications to
enhance compliance and minimise symptoms
Request pathology services and process pathology
results
Ensure compliance with the practice chaperone
policy
Support the team in dealing with clinical
emergencies
Provide advice on modification of lifestyle
issues, assisting and supporting patients in changing unhealthy behaviours
Provide contraceptive services, HRT check, offer STI and sexual health advice
Provide dressing and wound care including
leg-ulcer management, dopplers and compression bandaging
Perform venepuncture and electrocardiography
Carry out ear examination and irrigation
Deliver travel health care, including vaccinations
and advice

About us
We are a friendly, well established GP surgery in Leadgate, a small
village outside Consett, County Durham. We provide healthcare services to an
increasing list size of over 6000 patients. We are looking for an
experienced practice nurse to join our friendly practice team. We use SystmOne
and are a high QOF achieving practice with an interest in clinical research.
Our clinical team consists of 3 GP Partners, 1 salaried GP, 2 Advanced
Nurse Practitioners, a Practice Nurse, 2 Healthcare Assistants and a Clinical
Pharmacist supported by the management and administrative teams.
