Job summary

Primary care is the job for you if you are self-motivated, organised individual who is committed to providing high quality care to the community.

We have a new and developing role within primary care looking to utilise your specialist Occupational Therapy skills and experience supporting patients with mental health needs. 

This is a full-time post but with Job share and part time potential.

We are looking for a motivated, enthusiastic and professional individual with a good clinical background, who has the ability to prioritise workload, be organised and who is confident to work independently and as part of a team.

Main duties of the job

The Occupational Therapist role is new for our PCN and we are keen to harness the skills of this profession with a focus on improving the mental wellbeing for our patient population. We are looking for someone who will be able to work alongside other health professionals to support patients with a range of mental health needs. The successful candidate will offer support and brief interventions to our patient population and will be able to work autonomously, demonstrating competent decision making, based on sound clinical reasoning and knowledge. They will work closely with our established PCN social prescriber team and the PCN mental health lead GP and the voluntary sector to ensure patients receive high quality care and treatment to meet their needs.

We are a group of 6 practices covering the rural north and east of Winchester. The practices are at Gratton, Stockbridge, Whitchurch, Watercress, Alresford and West Meon.

We are looking for a dedicated individual to work with the PCN Practices to help provide integrated care inconjuction our GPs, Paramedics, Pharmacists, Social Prescriber, Care Coordinators and Medical Administrators.

 Improve the quality of patient care and service within the Primary Care Network. 

Practice autonomously in accordance with the local and Trust Policy, Protocols, Standards and Guidelines.

 Identify health inequalities and give advice as to reasonable adjustments that can be made in order to ensure services are accessible.

 Disseminate information about best practice and lead on the implementation of good practice that has been developed locally and nationally.

Provide compassionate care that is based on empathy, kindness, respect and dignity.

 Participate in the planning, development and evaluation of a primary care mental health service model Lead on defined projects

To offer support, guidance as appropriate to team members.
