Job summary

Now is the time to join us and help take forward an innovative and forward thinking Primary Care Network (PCN)

We are seeking a PCN Manager who will work alongside our Clinical Director and PCN board to drive the development of our network.

We want an individual who can lead in delivering a wide range of business and patient centred transformation initiatives across our area and provide operational support and leadership.

30 hours a week across 4 - 5 days.

The PCN was established to enhance patient services and improve the health of our 45,000 patient list size.

We are looking for a highly enthusiastic, self-motivated dynamic individual to fit in with our friendly team.

Organisation, leadership and flexibility are key qualities required for this role.

Primary Care Networks are relatively new and this role will adapt and grow on as the PCN concepts develops, therefore for the candidate must have a positive and open mind to change.

We are looking for a Manager to support the Clinical Director in driving the PCN forward, recruit and manage ARRS staff and manage the demands of the enhanced services.

The candidates should have experience of working with clinicians and ideally in primary care, be good at building relationships and working with a wide range of stakeholders, and be able to make change happen.

Main duties of the job

The Primary Care Network Manager role will directly support and work with the PCN Clinical Director and member practices to support and facilitate the implementation of services in line with Network Contract Directed Enhanced Service specifications, and any other initiatives the PCN is pursuing.

The postholder will facilitate the development of cross sector partnership working between the PCN and other key stakeholders including commissioners, community services, acute trusts, local councils, mental health, community groups and 3rd sector organisations.

We are 6 practices surrounding Winchester forming the Winchester Rural North & East PCN. West Meon, Alresford, Watercress, Gratton, Stockbridge and 2 Rivers.

We have an establisehed and developing a team across the network of clinical pharmacists, Pharmacy Techinicians, Care Coordinators and Social prescribers.

To play a pivotal role in the delivery of high quality primary health care within Winchester Rural North & East PCN

Support the PCN Clinical Director to ensure the smooth running of the Network.

To be involved in the management and development of the Network

To be responsible for the production of performance and quality reports within the Network and other Network initiatives

To be responsible for the oversight of the Network financial systems/payments to all the practices within the Network, working alongside the Lead practice and their financial infrastructure

Ensure the PCN is compliant with the GDPR and FOI along with other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance

To represent the PCN in cross organisation meetings when agreed e.g. locality/CCG meetings.
