We are looking to recruit a forward thinking, clinical pharmacist interested in developing their career working primary care improving patient outcomes related to medicines and ensuring patient safety.
We are the Winchester Rural North Primary Care Network working with practices from West Meon to Stockbridge and we are looking for Clinical Pharmacists in a full-time capacity. Part time will be considered for the right individual.
The Clinical Pharmacist role is very much patient-focused and there will be plenty of opportunity to utilise clinical skills to improve the health of the local community. This will include managing long-term conditions, supporting care home patients and hospital discharge, prescribing arrangements, providing specific advice for those on multiple medications and supporting the GP practices. You will act as a specialist resource to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals as required.

Main duties of the job
We are looking for
a motivated, enthusiastic and professional individual with a good clinical
background, who has the ability to prioritise workload, be organised and who is
confident to work independently and as part of a team. The role will include
working with, and overseeing the work of, Pharmacy technicians. Applicants will
be computer literate, have excellent communication skills, and be able to
travel within the Network area for service provision.

Gratton, Stockbridge, Whitchurch, Watercress, Alresford and West Meon). We are
a cohesive, mutually supportive group and we welcome the opportunity to bring
in new roles to work with our current teams. All of our surgeries are
dispensing practices, who work with the current CCG medicines management teams
who will help provide support and training.

The post holder will take responsibility for the care management
of patients with chronic diseases and undertake clinical medication reviews to
proactively manage people with complex polypharmacy, especially the elderly,
people in care homes, those with multiple co-morbidities (in particular
frailty, COPD , asthma, depression) and people with learning disabilities or
autism to implement the structured medication review and optimisation set out
in the Primary Care Network Contract Direct Enhanced Service (DES).
They will provide specialist expertise in the use of medicines
whilst helping to address both the public health and social care needs of
patients at the PCNs practices and to help in tackling inequalities.
They will provide leadership on person centred medicines
optimisation (including ensuring prescribers in the practice conserve
antibiotics in line with local antimicrobial stewardship guidance) and quality
improvement, whilst contributing to the quality and outcomes framework and
enhanced services. Through structured medication reviews (SMRs), they will support
patients to take their medications to get the best from them, reduce waste and
promote self-care.
They will have a leadership role in supporting further
integration of general practice with the wider healthcare teams (including
community and hospital pharmacy) to help improve patient outcomes, ensure
better access to healthcare and help manage general practice workload.
They will develop relationships and work closely with other
pharmacy professionals across PCNs and the wider health and social care system.

Essential
- Masters degree in pharmacy (MPharm) (or equivalent) Registration with the General Pharmaceutical Council Independent prescriber or working towards/intent of gaining independent prescribing qualification
Experience
