Job summary

An exciting opportunity has arisen for an enthusiastic, committed and caring Paramedic Practitioner to join our friendly team at Forest hill road group practice, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, headaches and mental health crisis etc.

You will be working as part of a clinical team with GP's, Practice Nurses and supporting Healthcare Assistants.

Salary – Will be dependent upon experience.

Main duties of the job

The successful post holder will be acting within their professional boundaries to support the GP's through holding clinics and may be expected to visit patients within their own home.

  • Undertaking triage of patients.
  • Operating under the supervision of a GP; provide general medical care, assessments, evaluation, referral and treatment for patients in clinics.
  • Provide direct clinical care to patients using established clinical guidelines.
  • Assess patients presenting with acute illness or for review of chronic disease conditions.
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and follow up as appropriate.
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
  • Undertake home visits for acute illness or chronic disease management.
  • Work flexibly to meet the needs of patients.
  • To undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder.

We are dynamic, forward looking Practice with 4 Partners, 4 Salaried GPs, Advance Nurse Practitioner, 2 Practice Nurses, Advanced nurse practitioner, HCA, in house Pharmacist supported by our Receptionists and Administrative Team. 

We are a Practice that performs well in QOF and other national and local Agreed Services. We currently hold a Good rating with the CQC. The Practice is currently looking to recruit a friendly, enthusiastic and hard-working Paramedic practionier to join our team. You will work with the practice team to meet the needs of patients, nurses and GPs following policies.

The primary care paramedic practitioner will work closely with our GP's to advise on treatment and healthcare for patients, to include those presenting in the primary care setting and those in the community. 

The post holder will attend home visits to include residential and nursing homes, where they will liaise with staff, family and other stakeholders in the production of care plans to be shared with other providers, to include the Out of Hours (OOH) service. 

The practitioner will also triage acute patients presenting in the surgery environment. They will demonstrate safe, clinical decision-making and expert care for patients. They will work collaboratively with the multi-disciplinary team to meet the needs of our patients.
