Job summary
An exciting opportunity has arisen for an
enthusiastic, committed and caring Paramedic Practitioner to join our friendly
team at Forest hill road group practice, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, headaches and mental health crisis etc.
You will be working as part of a clinical
team with GP's, Practice Nurses and supporting Healthcare Assistants.
Salary – Will be dependent upon experience.
Main duties of the job
The successful post holder will be acting within their professional boundaries to support the GP's through holding clinics and may be expected to visit patients within their own home.
- Undertaking triage of patients.
- Operating under the supervision of a GP; provide general medical care, assessments, evaluation, referral and treatment for patients in clinics.
- Provide direct clinical care to patients using established clinical guidelines.
- Assess patients presenting with acute illness or for review of chronic disease conditions.
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and follow up as appropriate.
- Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
- Undertake home visits for acute illness or chronic disease management.
- Work flexibly to meet the needs of patients.
- To undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder.

The post holder will attend home visits to include residential and nursing homes, where they will
