Job summary
An exciting opportunity has arisen for a Care Co-ordinator to join our friendly team at Abbey House Medical Practice.
This new role will involve working closely with GPs and other primary care professionals within the practice and the Primary Care Network to identify and manage a caseload of identified patients. As a Care Co-ordinator, you will ensure that the appropriate support is made available to these patients and their carers in addition to ensuring that their changing needs are addressed.
This is a permanent position and the hours are negotiable - Full or part-time flexibility up to a maximum 37.5 hours per week.
Main duties of the job
We are seeking an individual who is enthusiastic, compassionate, approachable, organised and must be able to work as part of a team. The successful candidate must demonstrate good communication skills in order to provide timely and accurate verbal and written communication to patients, carers and a wide variety of healthcare professionals. Problem-solving skills and successful time management are also essential in the undertaking of this role.
IT experience is essential (MS Office & Internet) and a knowledge of SystmOne would be an advantage however full training will be given.
Previous experience of working in a "people-centered environment" is preferred and knowledge of the local area is desirable, along with an awareness of the challenges faced in the community.
The successful candidate must have or must be willing to complete Safeguarding Certification & training set out by the Personalised Care Institute.

We are offering this post as a permanent position and the hours are negotiable - We can offer full or part-time flexibility up to a maximum 37.5 hours per week.
Hourly rate £9-11 per hour dependent on experience.
