Job summary

An exciting opportunity has arisen for a Safeguarding Care Co-ordinator to join our friendly team at Abbey House Medical Practice.

This new role will involve working closely with GPs and other primary care professionals within the practice and the Primary Care Network to identify and manage a caseload of identified patients. As a Safeguarding Care Co-ordinator, you will ensure that the appropriate support is made available to these patients and their carers in addition to ensuring that their changing needs are addressed.

This is a permanent position and the hours are negotiable - Full or part-time flexibility up to a maximum 37.5 hours per week.

Main duties of the job

We are seeking an individual who is enthusiastic, compassionate, approachable, organised and must be able to work as part of a team. The successful candidate must demonstrate good communication skills in order to provide timely and accurate verbal and written communication to patients, carers and a wide variety of healthcare professionals. Problem-solving skills and successful time management are also essential in the undertaking of this role.

IT experience is essential (MS Office & Internet) and a knowledge of SystmOne would be an advantage however full training will be given.

Previous experience of working in a "people-centered environment" is preferred and knowledge of the local area is desirable, along with an awareness of the challenges faced in the community.

The successful candidate must have or must be willing to complete Safeguarding Certification & training set out by the Personalised Care Institute.

We are offering this post as a permanent position and the hours are negotiable - We can offer full or part-time flexibility up to a maximum 37.5 hours per week.

Hourly rate £9-11 per hour dependent on experience.

RESPONSIBLE TO: Safeguarding Lead GPs / Clinical Administrator (Clinically) & Deputy Practice Manager (Personnel)

RESPONSIBLE FOR: Working closely with GPs and other primary care professionals within the Practice and PCN to identify and manage a caseload of identified patients. To make sure that appropriate support is made available to these patients and their carers and ensuring that their changing needs are addressed.

DUTIES TO INCLUDE

1. Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids.

2. Bring together all of a persons identified care and support needs, and explore their options to meet these into a single personalised care and support plan, in line with best practice.

3. Help people to manage their needs, answering their queries and supporting them to make appointments, referring them back to other health professionals where appropriate.

4. Work with safeguarding professionals, to assist with the completion of safeguarding forms and case conference paperwork.

5. Taking an administrative lead with safeguarding for adults and children.

6. Organising the practice safeguarding policies and keeping the practice intranet up to date in this respect.

7. Take charge of the annual safeguarding audit.

8. Take an administrative lead in PREVENT policies and queries in the practice.

9. Provide coordination and navigation for people and their carers across health and care services, alongside working closely with the care co-ordinator, social prescribing link workers, clinical administrator and other primary care roles.

10. Support the coordination and delivery of safeguarding meetings and MDTs.

11. Ensure that all activities are recorded, monitored and evaluated.

12. Attend all meetings as required.

13. Work in accordance with practice policies, ensuring that all work and procedures are undertaken in accordance with issued guidelines.

14. Attend any training courses and supervision sessions as required.

15. Promote the professional image and services offered by the surgery.

16. Any other duties that may be required and agreed by the Doctors and Management Team
